Question about substitutions:

If I place this type of order:

1 -- player is slightly injured -- first from the bench -- player John -- on position 5

But the player is badly injured (not Slightly) and leaves the field, will the substitution take place anyway, or do I also need to have this order in place:

1 -- red card or no player -- first from the bench -- player John -- on position 5

...for the same player?

That is enough for both injuries.

1 -- red card or no player -- first from the bench -- player John -- on position 5

If the player gets red this player will come on his place. Another player will be changed and he will came up the field but at position 5

2. if player is unable to continue because of an injury he is replaced automatically even with no order with the first available on the bench. I dont know if you can replace him by other (non default) player. Also you can reshuffle players on the field by this condition if you had 3 substitutions at the moment and you continue playing in 10 (or less ;))

Yes. At least MID players are suppoused to pass more often.
